29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.    a) Signage lights outside car repair shop:    An employee was utilizing an articulating boom aerial lift van to repair signage lights. Employee was not protected against electric shock by de-energizing the lights, or wearing personal protective equipment with insulation. On or about 05/15/18.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.    Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    a) Signage lights outside car repair shop:    An employee was utilizing an articulating boom aerial lift van to repair signage light. The employee did not wear a body belt for fall restraint or a safety harness for fall arrest, and was exposed to falling out of the aerial lift bucket without fall protection. On or about 05/15/18.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.       a) Signage lights outside car repair shop:       The employer did not provide training to an employee who was utilizing an articulating boom aerial lift van to repair signage light. Employee could not recognize hazards associated with using aerial lifts, and was exposed to hazards, including but not limited to: working on an aerial lift without fall protection. On or about 05/15/18.
