29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program.    Employees engaged in welding steel decking on a commercial building located at 6603 W. 135th Street, Overland Park, Kansas were exposed to a fall hazard of approximately 13 feet due to the safety harness being attached to a twisted nylon/polyester rope, that was tied into a knot.  The anchorage component of a twisted nylon/polyester rope did not provide an adequate fall protection system.
