29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On May 15, 2018, employees performing roofing activities without fall protection were exposed to fall hazards of approximately 30 feet to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about May 15, 2018, the employer did not ensure employees safely used the ladder to safely work on a commercial roofing project. The ladder was not secured at the top and did not extend three feet above the landing surface. Employees wee exposed to a fall hazard up to 30 feet to the ground below.
