29 CFR 1926.300(b)(1): When power operated tools are designed to accommodate guards, they were not equipped with such guards when in use: A. On or about 08/14/18, on the 7th floor of 132-27 39th Ave. Flushing, NY Employee was cutting metal/tiles with a Makita 241 4B cut-off machine without a guard. ABATEMENT VERIFICATION IS REQUIRED.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: A. On or about 5/15/18, scaffolds at 132-27 39th Ave. Flushing, NY Employee working from a tubular welded frame scaffold with the personal fall arrest system was tied to the frame of the scaffold itself. There was only three sections of the scaffolds. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
