29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about May 8, 2018, and times prior thereto, at the Rotary Car Unloader Building Supply Fan, where the employer did not ensure employees isolated the stored energy to the Aerovent Backward Inclined Utility Blower (BUIB), Model #33 BUIB, Serial #J-3953296-2-1 prior to verifying tension of the dual v-belts on the motor drive powering the blower.
