29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  For Citation 1, Item 1, issued on 6/11/2018 with an abatement date of 7/9/2018, the employer did not certify the complete abatement of the cited violation.

29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  For Citation 2, Item 1, issued on 6/11/2018 with an abatement date of 7/9/2018, the employer did not submit documents demonstrating complete abatement of the cited violation to OSHA.

29 CFR 1926.1053(b)(1): Portable ladders are used for access to an upper landing surface and the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    The employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing.  Workers performing residential construction were exposed to falling approximately twelve (12) feet while using an extension ladder to ascend/descend from the roof to the ground.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    The employer did not ensure fall protection was implemented during residential construction operations when workers were exposed to the hazard of falling approximately twelve (12) feet off the roof eve when they were installing roof shingles.  Xtreme Xteriors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1215855, citation number 1, item number 1 and was affirmed as a final order on 3/30/2017, with respect to a workplace located at Lot #90, Bradstreet Drive, O'Fallon, IL, 62269.  Xtreme Xteriors, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1217788, citation number 1, item number 1 and was affirmed as a final order on 5/02/2017, with respect to a workplace located at 2629-2631 Eastview Dr, Belleville, IL 62221.
