29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a. On or about 04/27/2018 @ 11:45am at 2411 Clement Rd. Lutz, FL 33548, an employee suffered a work-related injury and was hospitalized. The employer was aware of the in-patient hospitalization on 04/28/2018. The employer notified OSHA on 05/01/2018 at 3:30 p.m. of the in-patient hospitalization.
