What this inspection record means

OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

Shear connectors (such as headed steel studs, steel bars or steel lugs), reinforcing bars, deformed anchors or threaded studs were attached to the top flanges of beams, joists or beam attachments so that they projected vertically from or horizontally across the top flange of the member prior to the metal decking, or other walking/working surface, was installed. On or about 5/17/2018, employees from a Clayco, Inc. subcontractor, Weaver Steel Construction L.L.C., were performing structural steel work on a commercial building located at 7600 Forsyth Avenue in Clayton, MO. While connecting structural steel members for flooring and hand rails for perimeter cables, the employees were required to walk on perimeter beams, which had window boxes previously installed by the steel manufacturer. The window boxes had rebar attached to them which projected horizontally across the top of the structural steel member creating a tripping hazard. The boxes were installed prior to metal decking or other walking/working surfaces.
