29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a. Generative Growth II, LLC - Van Wert, Ohio: On or about May 17, 2018, the employer did not ensure that the Yale electric pallet jack (model MPB040-EN24T2748; s/n B827N31252F) was removed from service during a time in which it was not in safe operating condition. Employees were exposed to electrical shock and burn injuries in that the cover over the battery terminals was removed. b. Generative Growth II, LLC - Van Wert, Ohio: On or about May 17, 2018, the employer did not ensure that the Big Joe forklift (model PDH-30-106; s/n 367050) was removed from service during a time in which it was not in safe operating condition. Employees were exposed to struck-by injuries from falling loads in that the lift presented a hydraulic leak that caused the mast to drift.
