29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound top rail capacity): a) Valley View Drive, Jessup, PA: The north end of the frame scaffold, at the ladder access point, was not equipped with a top rail which exposed employees to a fall of about 14 feet, on or about May 17, 2018. b) Valley View Drive, Jessup, PA: The south end of the frame scaffold, on the second level, was not equipped with a guard rail system which exposed employees to a fall of about 14 feet, on or about May 17, 2018. c) Valley View Drive, Jessup, PA: The side-bracket platform on the south end of the frame scaffold, at the space between two separate columns, was not equipped with a guardrail system, which exposed employees to a fall of about 16 feet, on about May 17, 2018. d) Valley View Drive, Jessup, PA: The frame scaffold installed across the front of structure was not equipped with midrails at various locations on the second (working) level which exposed employees to a fall of about 14 feet, on or about May 17, 2018. ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED
