29 CFR 1926.501(b)(3): "Hoist areas." Each employee in a hoist area shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems. If guardrail systems, [or chain, gate, or guardrail] or portions thereof, are removed to facilitate the hoisting operation (e.g., during landing of materials), and an employee must lean through the access opening or out over the edge of the access opening (to receive or guide equipment and materials, for example), that employee shall be protected from fall hazards by a personal fall arrest system.  On May 18, 2018, an employee was exposed to falls while signaling the crane operator when the employee was pushing the warning line closer than 6' to the roof edge.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.  On May 18, 2018, employees were exposed to falls while loading equipment onto the roof of a commercial building when the access path from the roof edge to the area surrounded by warning line was not established.

29 CFR 1926.1408(a)(1): Identify the work zone by either:  29 CFR 1926.1408(a)(1)(i): Demarcating boundaries (such as with flags, or a device such as a range limit device or range control warning device) and prohibiting the operator from operating the equipment past those boundaries, or  29 CFR 1926.1408(a)(1)(ii): Defining the work zone as the area 360 degrees around the equipment, up to the equipment's maximum working radius.  On May 18, 2018, employees were exposed to electrical shock while using a Terex TC4792 truck crane with 72' of boom extension when it was set up 42' away from a power line when the employer did not ensure steps were taken to prevent encroachment into the 12.4 kV power line.
