29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: On or about 5/18/2018 at 1975 Old Covington Road NE, Conyers, GA: The employees were required daily to enter the confined space of the waste truck ejector blade area to clean and clear waste material, without the employer evaluating the workplace to determine if a work area was a permit-required confined space.  The employer indicated that they have about eight (8) waste trucks that require the employees to enter the hazardous area.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about 5/18/2018, at 1975 Old Covington Road NE, Conyers, GA: An energy control program with specific energy control procedures were not developed where employees were required to perform repairs, maintenance, service, and troubleshoot the waste truck equipment.   a. Employee was observed changing a wheel on a waste truck without locking or tagging out the equipment.  b. Employee(s) not provided specific written energy control procedure before entering a permit-required confined space.

Safety data sheets were provided during the walk-around inspection.
