29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Jobsite, 3947 Liberty Avenue, Pittsburgh, PA:  On or about May 16, 2018 an employee engaged in roofing activities approximately 30 feet above the ground was not protected from falling by a guardrail system, personal fall arrest system or safety monitoring system.   The employer, MLG Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1130866, citation number 1, item number 1a and was affirmed as a final order on April 5, 2016, with respect to a workplace located at Canterbury Court, 177-183, West Chestnut Street, Washington, PA.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a) Jobsite, 3947 Liberty Avenue, Pittsburgh, PA:  On or about May 16, 2018 the employer had not prepared a written certification record showing that employees were trained on the recognition of fall hazards.   The employer, MLG Contracting, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 1130866, citation number 1, item number 1b, and was affirmed as a final order on April 5, 2016, with respect to a workplace located at Canterbury Court, 177-183 West Chestnut Street, Washington, PA.
