29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (a) Facility, 600 Boyce Road, Pittsburgh, PA: On or about May 18, 2018 the employer has not provided devices or hardware for employees to use for isolating the blenders from the energy sources during cleaning and/or maintenance procedures. Employees climb inside the blenders to complete the cleaning process and no devices or hardware has been provided to secure the equipment from energy sources.

29 CFR 1910.147(c)(7)(i): The training program of the employer did not include all the training elements listed under Items A-C of 29 CFR 1910.147(c)(7)(i): (a) Facility, 600 Boyce Road, Pittsburgh, PA: On or about May 18, 2018 the employer did not have an energy control program that included training for authorized, affected and other employees. The employer did not provide training to employees who would be classified as authorized in that employees who service or maintain equipment had not been provided copies of the specific lockout procedure or equipment necessary to control energy sources. Employees who work in the immediate area who would be classified as affected had not been provided training to ensure they understood what a lock or tag signified and the prohibition relating to attempting to restart or reenergize machines or equipment.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Facility, 600 Boyce Road, Pittsburgh, PA: On or about May 18, 2018 energy isolating devices were not used to control the electrical and pneumatic sources for the blenders when employees are performing cleaning and servicing operations.
