29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) East and west side of framed house - On or about May 15, 2018 employees framing the raised house gained access to the first floor by climbing up and down a step ladder that was closed and leaning against the exterior framed floor and did not extend 3 feet past nor was it tied off.
