29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person.    a) At the jobsite, on the roof, on or about 5/16/18:  Workers were attached to anchors that were not secured with the correct number of fasteners according to the manufacturers instructions.  An anchor was also installed in an incorrect orientation to the point of use.      b)  At the jobsite, on or about 5/16/18:  Interviewed employees, were not able to describe the correct number of fasteners to use.  The Foreman was not on site, and the crew lead (translator) was also not able to describe the correct number of fasteners to use in this application.
