OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) On or about 05/21/2018, on the second floor mezzanine of Concourse B at La Guardia Airport, Flushing. NY Employees were using a portable cable puller machine (Serial No. ANB01007) along with a hook sheave and pulley to pull cable through conduit. An employee was struck by the pulley when it broke free from its fastener during a cable pull.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 05/21/2018, on the second floor mezzanine of Concourse B at La Guardia Airport, Flushing. NY Employees working with Greenlee Ultra Tugger Portable Cable Puller machine (Serial No. ANB01007) with the pulley (hook sheave) model 651 for guiding the pulling rope were not trained. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
