29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE. The Hectate - Pacifica, CA: On May 3, 2018, two employees attempting to salvage a beached sailboat were either not wearing or provided with adequate PPE. Employees were tossed into rough surf when hit by a large wave; one employee was not wearing a personal flotation device (PFD) and neither were wearing head protection such as bump camps. Employees were exposed to drowning hazards.

29 CFR 1915.152(e)(3): Employees who did not understand or display the skills to use personal protective equipment properly were not re-trained: The Hectate - Pacifica, CA: On May 3, 2018, an employee attempting to salvage a beached sailboat did not wear a U.S. Coast Guard approved personal flotation device (PFD) in accordance with the employer's written Job Safety Assessment when entering rough surf and boarding the vessel thereby exposing him to drowning hazards. In accordance of with paragraph (e) (2) of this section, retraining shall be conducted.
