On or about May 10, 2018, employees were not protected from falling while working/walking on a roof greater than forty two feet above lower level and at time sixty seven feet.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location - 147 Rangeway Road, Billerica, MA 01821. On or about May 10, 2018, employees were not protected from falling when using a personal fall protection systems that were not attached correctly to support at least 5000 pounds. Employees were working/walking on roof structure between forty two to sixty five feet above lower level.
