29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On 5/3/18, at Lanier Islands Legacy Lodge, employees were permitted to operate powered industrial trucks without first completing a powered industrial truck training program. On April 25th, 2018 an employee was operating a diesel sit down forklift and flipped the truck incurring foot injuries during the event.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation. On 5/3/18, the employer was not recording a description to the extent the regulation required in Column F on the OSHA 300 and instead was vague in the description on entries made in February, March and April of the 2018 OSHA 300 log. Descriptions included: "Dog bit employee on right leg", "lifting racks of glasses and injured left wrist".
