29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition:  a. On or about 5/22/2018, at 1200 S Swinton Ave Delray Beach, FL 33444, the Komatsu forklift (SN: 555267A) had an inoperable horn; exposing the employees to a possible struck-by hazard.  b.On or about 5/22/2018, at 1200 S Swinton Ave Delray Beach, FL 33444, the Toyota forklift (SN: 8EFG6025) had an inoperable back up alarm; exposing the employees to a possible struck-by hazard.
