29 CFR 1910.1200(g)(8): The employer did not ensure that copies of the required material safety data sheet for each hazardous chemical were readily accessible during each work shift to employees when they were in their work area(s): On or about May 21, 2018, and at times prior thereto, employees were exposed to health hazards while using corrosive chemicals such as, but not limited to, Terraclor 75WP, containing pentachloronitrobenzene, a toxic chemical, without safety data sheets being readily accessible.

29 CFR 1910.141(c)(1)(i): Except as otherwise indicated in this paragraph (c)(1)(i), toilet facilities, in toilet rooms separate for each sex, was not be provided in all places of employment in accordance with table J-1 of this section: On or about May 21, 2018, and at times prior thereto, employees were exposed to health hazards while working in the field without toilet facilities readily available for increased use affected by environmental factors such as heat and by high fluid intake.
