29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  a) Sunder, Inc. at 2100 Alpine Drive, Vail, CO 81657:  On and before May 22, 2018, the controlling employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person.  The employer did not ensure that excavation subcontractor work was being carried out in a safe manner and that protective systems were in place to protect employees from cave-in hazards.
