29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. In the Feed Mill, an employee engaged in rolling corn at the roller mill was exposed to amputation hazards, in that the energy control procedure was not clear or specific on how to shut down and secure the roller mill.

29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition: At the grain elevator facility, employees operating a Hyster 50, forklift, (Model H60XM, SN H177B29173Y) and employees near forklifts in operation, at the facility were exposed to struck-by hazards. The employees were exposed in that the control lever(s) for the forks were missing the control markings.
