This exposed employees to struck by hazards.

On or about May 21, 2018 and at times prior thereto, the employer did not ensure that a job made lifting device was adequately secured to the mast of the powered industrial truck. This exposed employees to a fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about May 21, 2018 and at times prior thereto, the employer did not provide fall protection for employees walking/working from the third floor of the building retrieving material from the edge of the floor. This exposed employees to a fall hazard greater than six (6) feet to the ground below.
