29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck, the employer did not ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5). On or about May 21, 2018 and at times prior thereto, the controlling employer permitted an employee, of GLZ Acoustics, LLC, to use a powered industrial truck and did not verify the operator had successfully completed the required training. This exposed employees to struck-by and caught-in hazards.
