29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them:  (a) 888 1st Street, NE, DC: Rooftop, Between the North and South Cooling Towers - On or about May 22, 2018, the employer failed to ensure that the 270 VAC heat tape on the cooling tower was de-energized prior to replacing the liquid tight conduit.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
