29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems.   JOBSITE:  The employer did not ensure that the employee engaged in activities on the roof was protected from the hazard of falling to lower level or ground by the use of appropriate types of personal fall arrest devices or other type of systems.  On/or about May 05, 2018, broken concrete blocks and bricks fell onto the employee who was working/walking on the roof on the Front Building at Connecticut Precast Corporation to estimate materials and do other duties required for the project, and the employee fell to the ground and sustained injuries for the incidents.
