29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Kitchen - On or about May 19, 2018, an employee reached into a pasta extruder machine (PMR 96 Bottene) while it was running which resulted in an in-patient hospitalization. The incident was reported on May 21, 2018 to OSHA. It was not reported by the employer.
