29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: 2155 Caton Ave On or about: May 22, 2018 a) An employee working at the edge of stairwell landing that was not equipped with guardrails nor was the employee wearing fall protection. The employee was installing framing around the stairwell landing on the roof.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 2155 Caton Ave, roof On or about: May 22, 2018 a) Employee installing framework on the edge of a stairwell landing on the roof, that was not equipped with a guardrail and worker was not wearing a personal fall arrest system. Employee exposed to a fall of approximately 10 feet.
