29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employees working from an aerial lift:  (a) New commercial construction located at 1415 Lafayette Pkwy., LaGrange, GA: Front side: On or about 05/22/2018, and at times prior, the employer exposed employees to ejection/fall hazards from approximately 25-feet from the lower level, in that a fall arrest system was not provided while install aluminum panels from a JLG 400S Aerial lift basket.
