29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On or about May 22, 2018, on the east exterior elevation of a building under construction located at 2101 N. Australian Avenue in West Palm Beach, Florida, an employee performing concrete form installation work on a bracket platform was exposed to a fall of approximately 40 feet from the platform that was not inspected for visible defects by a competent person before each work shift and prior to allowing employees to work from it.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about May 22, 2018, on the east exterior elevation of a building under construction located at 2101 N. Australian Avenue in West Palm Beach, Florida, an employee was exposed to a fall of approximately 40 feet while performing concrete form installation work from a bracket platform which did not have guardrails installed on all open sides and ends of that platform.
