29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: a) On or about May 15, 2018 and at times prior, two employees were exposed to possible electric shock while accessing the electrical panel and switching the electrical circuit breakers on and off. The energized electrical circuit breaker cabinet was not installed with a panel board cover.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems; or in the alternative, 29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about May 15, 2018 and at times prior, three employees were exposed to fall hazards of approximately 10 feet while working near a walkway at the second level with an unprotected edge or a hole of approximately 2 X 10 feet. The unprotected edge or hole was not provided with a fall protection system or a cover respectively.
