29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  A- Commercial Roof: An employee was exposed to fall hazards of approximately 8ft to the concrete below as an A Frame ladder was used as a means of roof access. Condition noted on or about 05/10/2018  B-Commercial Roof: An employee was exposed to fall hazards of approximately 15ft as an extension ladder was anchored to the 10 on 12 pitch being used as a means of fall protection. Condition noted on or about 05/10/2018
