29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l)  (Construction Reference 1926.602):    a)  4 Hanover Avenue, Florham Park, NJ and at the Corporate Offices at 602 Tillman Street, Hillside, NJ.    By not ensuring that those operating the JLG Lull rough terrain forklift had the training and evaluation to safely operate it, the employer placed worker(s) in danger, especially those being elevated in a makeshift box on the forks.                                                 Violation observed on or about 07/16/2018.

29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a)  4 Hanover Avenue, Florham Park, NJ.    Worker was exposed to injury from but not limited to falls of 28 feet from the makeshift wood scaffold platform that was elevated by the forks of the rough terrain forklift where the platform was not attached to the fork and the forklift was moved horizontally while the worker was still in the platform.                                                  Violation observed on or about 07/16/2018.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  4 Hanover Avenue, Florham Park, NJ.    Workers were exposed to falls of approximately 28 feet to the ground below as the extension ladder used to access the roof did not have its side rails extend at least 3 feet above the upper landing surface at the eave of the roof.                                              Violation observed on or about 07/14/2018.
