Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) Jobsite, 1036 Brookline Boulevard, Pittsburgh, PA: On or about May 23, 2018 an employee engaged in roofing activities on a low sloped roof with an unprotected side or edge was not protected from falling approximately 20 feet by the use of guardrails, safety nets, personal fall arrest systems or warning line system.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): (a) Jobsite, 1036 Brookline Boulevard, Pittsburgh, PA: On or about May 23, 2018 employees who were exposed to fall hazards when working on the low sloped roof had not been trained by a competent person to ensure the employee understood what fall hazards existed at the work site and what procedures were to be used to erect, maintain and inspect fall protection systems.
