29 CFR 1926.20(b)(1): The employer did not initiate and maintain programs necessary to ensure compliance with the Occupational Safety and Health Standards:                Employees were exposed to struck-by and fall hazards.  Employees engaged in residential construction activities were exposed to fall hazards greater than 20 feet and struck-by hazards from untrained fork truck drivers.  The employer did not provide employees and temporary employees with safety and health programs to prevent injury and illness in the workplace.  A safety and health program with policy, hazard assessment, and work site rules was not developed and/or implemented at this job site.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Employees were exposed to fall hazards.  Employees unloading doors from a forklift onto a third floor balcony were not provided fall protection.  The fall distance from the balcony to the ground below was 20 feet.
