29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) This violation occurred on or about May 22, 2018, at the job site located at 3100 Northside Blvd, Richardson, TX 75080, at the Northside II Apartment Complex, building #6, on the fourth floor, on the Schindler model 3300 simplex elevator, where employees working, adjusting, and connecting energized auxiliary wires were exposed to an electrical hazard.

29 CFR 1926.416(a)(3): Before work is begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists. The employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken: (a) This violation occurred on or about May 22, 2018, at the job site located at 3100 Northside Blvd, Richardson, TX 75080, at the Northside II Apartment Complex, building #6, on the fourth floor, on the Schindler model 3300 simplex elevator, where employees working and connecting auxiliary wires did not ascertain by instruments and were exposed to an electrical hazard.
