29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    (a) This violation occurred on or about May 22, 2018, at the work place located at 1010 Pilgrim Street, Mount Pleasant, TX at the Protein Plant Area, where employees working and removing metal pieces from the Lamella meat pump unit, type HM35.03R, part number HM35-0033 unit were exposed to an amputation hazard when the cover to the pulleys was removed.    In an alternative:    29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) This violation occurred on or about May 22, 2018, at the work place located at 1010 Pilgrim Street, Mount Pleasant, TX, at the Protein Plant Area, where employees working and removing metal pieces from the Lamella meat pump unit, type HM35.03R, part number HM35-0033, were exposed to an amputation hazard. The employer did not assess the potential flow of the meat process for employees working and removing metal from the meat pump unit.
