29 CFR 1926.501(b)(4)(ii): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Located in Building C, 4th floor, new sheet rock area, on May 23, 2018, employees were exposed to a 4" diameter floor hole. b) Located in Building C, 4th floor near elevator closet, on May 23, 2018, employees were exposed to a 4" diameter floor hole. c) Located in Building B, 2nd floor near plumbing shaft, on May 23, 2018, employees were exposed to a five and 1/2 inch wide by 5 feet floor hole.
