29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Saw Department., Kaltenbach Cold Saw - On or about May 16, 2018 through May 24, 2018 employees who performed cleaning of the saw area were exposed to amputation/laceration hazards from contact with blade while it came to a stop (residual motion). Employees inserted their hands and forearms into the saw blade housing, under the table, to remove metal chips with compressed air and were not required to control hazardous energy in the following sequence prior to cleaning: 1) turn off and shut down the machine, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) verify that isolation and deenergization of the machines or equipment have been accomplished. Alternative protection, including machine guarding, was not utilized. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Saw Dept., Kaltenbach Cold Saw - On or about May 16 through May 24, 2018 employees who performed servicing and maintenance tasks, including cleaning of the saw blade area, were exposed to amputation and laceration hazards. The employer failed to develop and document lockout/tagout procedures to protect employees during cleaning of the saw blade area where they were exposed to hazardous residual energy during the wind-down time for the blade to stop moving. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) Saw Department - On or about May 16 through May 24, 2018, employees who performed cleaning around the blade of the Kaltenbach Cold Saw were exposed to injury from the hazardous residual motion of the blade and unexpected energizing. The employer did not adequately train employees to ensure that they acquired the knowledge and skills required for the safe application and use of energy controls (energy isolation, lockout and verification) prior to removing the cover/guard on the saw and attempting to clean metal chips from around the blade area. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
