This was most recently observed at 911 Loyd Ct, Wayne, Nebraska on May 24, 2018 when employees were observed installing shingles to a residential structure roof and they were not using fall protection. The eve height was measured at 10'5" and the roof pitch was 6:12.

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder with more than one support attachment were not placed with the two rails supported equally: The employer is failing to protect employees from fall hazards while using a ladder to perform roofing operations. This was most recently observed at 911 Loyd Court, Wayne, Nebraska on May 24, 2018 when employees were observed using a ladder that was positioned against the sloped part of a pitched roof. The ladder was unstable because the upper rails were not supported equally which resulted in an increased the potential for the ladder to tip over.
