Location: 850 Flatbush Ave, Brooklyn, NY; Ground floor. On or about: 5/21/2018 a) An energized breaker panel was not provided with a cover. Unprotected live electrical parts were exposed in an active work area. b) Two of the sockets of the temporary lighting system were empty without a light bulb. Live electrical parts were exposed to employees for accidental contact.
