29 CFR 1926.501(a)(2): The employer did not determine if the walking/working surfaces on which its employees were to work had the strength and structural integrity to support employees safely: On or about May 22, 2018, during the removal/replacement of concrete parking deck material at a job site located at 253 Arlington St, Youngstown, Ohio 44512; the employer did not ensure that the walking/working surface had the strength and or structural integrity to support the employee as material was being removed with a pneumatic air driven hammer exposing the employee to a fall hazard of approximately 9.8 feet to the surface below.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about May 22, 2018, during the removal/replacement of concrete parking deck material at a job site located at 253 Arlington St, Youngstown, Ohio 44512; the employer did not ensure employees were protected from falling through holes in the walking/working surface. The employees were not protected by personal fall arrest systems, covers, or guardrail systems erected around such holes exposing the employees to a fall hazard of approximately 9.8 feet to the surface below.
