29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a)Maintenance Building close to Maintenance Office: On or about May 17, 2018 and times prior, employees used a 10 inch bench grinder to fabricate a bracket for a truck.  The left side tongue guard had a 1/2 inch opening between the abrasive wheel and the tongue guard.   b)Maintenance Building close to Maintenance Office: On or about May 17, 2018 and times prior, employees used a 10 inch bench grinder to fabricate a bracket for a truck.  The right side tongue guard had a 1 1/4 inch opening between the abrasive wheel and the tongue guard.   c) Maintenance Building near vehicle pits: On or about May 17, 2018 and times prior, employees used a Sioux Tools 10 inch ball bearing bench grinder to fabricate a bracket for a truck.  The right side tongue guard had a 3/4 inch opening between the abrasive wheel and the tongue guard.

29 CFR 1904.32(b)(3): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was not properly certified:  a) Starvaggi Industries, Inc: On or about January 29, 2018 and times prior, the Summary of Work-Related Injuries and Illnesses (OSHA 300A) was not properly certified in that the Human Resource manager signed the form.  The Human Resource manager is not a company executive.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) On or about May 17, 2018 and times prior, employees performed servicing and maintenance activities such as truck repairs and equipment repairs.  The employer did not perform annual periodic inspections of the energy control procedures to ensure the requirements of the energy control standards are met.
