29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: Excavation Site: On or about May 15, 2018, employees did not use a safe and acceptable means to locate an underground gas line that crossed the excavation path. This condition exposed employees to explosion hazards.
