Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Employee(s) performed flashing repairs, working at the west edge of a flat roof at 33 Randolph Avenue in Avenel NJ  without any means of fall protection; exposed to fall hazard of 20 ft.  Condition(s) were noted on or about 5/24/2018.

The extension ladder used for access to the roof was not extended at least 3 feet past the eave; exposing employee(s) to a fall of 20ft.   The conditions were noted on or about 5/24/2018.

The extension ladder used for roof access / egress was not subject to periodic inspection by a competent person, and featured an inwardly bent foot that could impact the ladder stability; exposing employee(s) to a fall of 20ft.
