29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about May 29, 2018, during the installation of a sanitary sewer line at a worksite located at 19149 East Bagley Road, Middleburg Heights, Ohio, the employer did not ensure a foreman working inside an approximately 6 foot deep portion of an excavation with a various depth of approximately 4 to 12 feet, was protected from cave-ins by an adequately designed protective system. The soil was classified as Type B .
