(b) On or about May 30, 2018 and at times prior to that date, for the employee exposed to amputation and/or caught-in hazards from reciprocating and moving machine parts while operating the Heidelberg printing press, company machine number 3040 in the letter press cell at 16 South 16th Street, Fargo, North Dakota.
