29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Main Casino Floor: The jacket on the extension cord, used to power the task lamp, was pulled away from the female end of the plug. Employees were exposed to electrical shock hazards due to no strain relief provided on the flexible cord.
